Preventive Health Clinic Services: Maternity Care Coordination (MCC) Target Population:
Program: MCC is a case management program with a focus on assisting the pregnant woman in meeting her psychosocial and medical needs as well as providing educational information relating to pregnancy and parenting. The case manager ensures the pregnant woman receives prenatal care and is a resource for services available in the community. The client is contacted as least monthly during the pregnancy and two months following delivery by a social worker or registered nurse. More frequent contact may occur based on the woman’s needs. Child Service Coordination (CSC) Target Population:
Program: CSC is a case management program designed to help children get the best possible care during the first years of life. There is no financial eligibility criteria for CSC and participation is voluntary. CSC is provided by registered nurses and social workers. CSC helps families identify their strengths and needs and find resources to make decisions. CSC coordinates all the services being received and assures that the rights of children and their families and protected. CSC monitors growth and development of children on the program and provides information/counseling on stages of development and parenting. Hours of Operation: Monday-Friday 8 am to 5 pm Exceptions are county recognized holiday/county closings for adverse weather. Contact:

Postpartum/Newborn Home Visit Target Population: Medicaid postpartum women and their newborns Program: The postpartum/newborn home visiting program is Medicaid funded home visiting program for residents of Rockingham County. The focus of the visit is an assessment by a registered nurse (RN) of the mother and her newborn, counseling, teaching and referral. The RN weighs the newborn and after the assessment, the RN reports any abnormalities to the client’s doctor. A home visit is attempted within five working days of hospital discharge. Hours of Operation: Monday-Friday 8 am to 5 pm A registered nurse is on call weekends and holidays from 8:00 am to 12:00 noon for visits which must be made prior to the next working day.
